Anesthetic Experience of a Patient with Craniofacial Impalement Injury by a Grinde - A Case Report -

Abstract
Craniofacial impalement injury is uncommon, but life threatening if brain is involved by foreign body. Grinder is widely used in work place. Because it wheels at high speed, if it is rushed out to human manipulating the machine, it may penetrate the body with high energy. A 46 year-old male patient was transferred to emergency room with impaled grinder into the midface. Grinder was impacted even into the sella turcica. He underwent craniectomy with grinder removal and facial bone reduction. He recovered slowly, but facial nerve palsy and blindness of left eye due to optic chiasm developed. The management principles of a patient with craniofacial impalement injury are to secure a safe airway, remain the impaling foreign body in situ during transportation, and maintain the hemodynamic stability during the removal of foreign body.
